Brotherhood Winery in New York can do customized labels. See their webpage on this site at:

There is a place in Cleveland, OH that will label for you or let you make your own wine and label it. It is called the Brew Keeper 216-292-2739. I had some done it looks good. They have full bottles and half bottles too.
