Personal Wine Notes Forms/ Web page

Hello, new to the board here. We are interested in finding forms that we can print on the internet 100 point scale or other that we can use to fill out ourselves. We are planning to eventually turn our notes along with others into our own little wine tasting notes webpage. Any ideas where to find these forms?
